肝本身沒有痛覺神經，但外圍包膜有。所以肝區疼痛有可能肝有問題(肝臟腫瘤腫大到壓迫到表面神經，隱隱作痛，感覺像是肌肉緊繃般的疼痛、或是脹痛)、吃飽就痛是膽囊炎(，尤其是油膩食物。急性膽囊炎會「劇烈疼痛」，慢性膽囊炎是「右上腹隱隱作痛」，所以很容易被誤會成肝病。)、疼痛部位偏下方可能是大腸腫瘤(大腸腫瘤壓迫到肝臟)和刺痛、間歇性疼痛可能是肋間神經痛。[1]
